Objective: This study aims to identify the clinical characteristics of Human Parainfluenza Virus (HPIV) infections in children, evaluate morbidity and mortality rates, and investigate the impact of chronic diseases on the course of these infections. Material and Methods: The research was conducted retrospectively and cross-sectionally on children aged 0-18 who tested positive for HPIV in the respiratory viral panel at Ankara Bilkent City Hospital Children’s Hospital between August 2019 and July 2023. Patients with multiple virus positivity were excluded from the study. Results: The study included 160 patients, of whom 61.2% were male and 38.8% were female. The most common presenting symptoms were cough (66.3%) and fever (52.5%). Of the patients, 41.2% were treated as inpatients, and 19.6% received care in the intensive care unit. The need for invasive or noninvasive mechanical ventilation was observed in 10% of the patients. There was a statistically significant association between the presence of lung infiltration and chronic disease with hypoxia (p &amp;lt; 0.001). Conclusion: While HPIV infections are generally mild, they can lead to significant morbidity and the need for intensive care in children with chronic diseases. These findings highlight the necessity for careful clinical evaluation and close monitoring of children with HPIV infections. The study’s results may contribute to the management and treatment strategies for HPIV infections in the pediatric population.

The Impact of Human Parainfluenza Virus on Child Health: A Clinical Study

人副流感病毒对儿童健康的影响：一项临床研究

OBJECTIVE:Type 1 diabetes mellitus (T1DM) is a polygenic disease influenced by genetic, environmental, immunological factors.There are few studies regarding siblings with T1DM.We aimed to evaluate the presentation, diagnosis, follow-up, sociodemographic characteristics of sibling T1DM cases. MATERIAL AND METHODS:We retrospectively reviewed characteristics of sibling cases followed with T1DM between January 2005 and May 2017. RESULTS: The prevalence of T1DM sibling diabetes in our clinic was 5.9%.We included 17 siblings (a total of 34 cases) who had diagnosis and follow-up data. One of the siblings was a twin.There were no statistically significant differences between the ages at diagnosis, presenting symptoms, duration of symptoms before diagnosis, glucose/C-peptide values at diagnosis, average HbA1c values in the first five years of follow-up, or hospitalization rates in the first five-years post-diagnosis between the first and second diagnosed siblings.Despite having a child diagnosed with T1DM, 23.6% of families had a second child diagnosed with diabetic ketoacidosis.Variations in antibody positivity were observed among siblings, there were no similarities between celiac disease, Hashimoto's thyroiditis.Vitamin D levels were significantly lower in siblings diagnosed secondarily. CONCLUSION:Our study is significant for being conducted at a reference center with a high number of diabetes patients under follow-up, for filling a gap in the literature with a detailed evaluation of sibling cases with T1DM.It serves as a comprehensive pilot study examining the manner, order of diagnosis, clinical, laboratory, and follow-up data of siblings with diabetes.There is a need for prospective studies with a larger number of sibling cases to further explore this topic.

Comprehensive Evaluation of Sibling Cases with Type 1 Diabetes

Vaccination is the cheapest, safest, and most successful public health approach to protect children's health and prevent infectious diseases. High vaccination rates ensure community immunity and prevent epidemics. A drop in immunization rates below 95% can lead to outbreaks of vaccine-preventable diseases, particularly measles, as well as increased morbidity and mortality. With the recent emergence of vaccine hesitancy (VH) and vaccine refusal (VR) concepts, especially in developed countries, the number of unvaccinated children is increasing both in our country and around the world. Vaccine hesitancy stems from many personal and environmental reasons, as well as sociocultural, environmental, economic, and political reasons. Lack of information about vaccines, fear of side effects, concerns about vaccine efficacy and safety, the idea that vaccines are harmful, anti-vaccine publications on the internet and social media, belief in natural immunity, and religious reasons are seen as the most common reasons for VH and VR in different studies. Raising awareness in society about the importance and necessity of vaccination, identifying the factors that lead to VH, and producing solutions are among the primary measures to be taken. Healthcare personnel play a very important role in the fight against vaccine hesitancy. It is important to establish good, effective, and trusting communication with vaccine-hesitant parents. Recently, in addition to vaccine refusal cases, the number of families refusing vitamin K and heel blood sampling has been increasing. Vaccine refusal, and refusal of health care services will increase neonatal and childhood morbidity and mortality. Legal measures should be taken to protect the best interests of the child. Valid and reliable scales that evaluate parents' vaccine acceptance and hesitancy will be a source of information in the fight against vaccine hesitancy.

Objective: Toilet training is an important step in the development of children. It was aimed to examine the factors affecting the toilet training of children aged 18-36 months and the effect of professional support on training process. Material and Methods: In the study, data was collected through a questionnaire. Quantitative method was used as the research method in the questionnaire. 214 children were divided into 2 groups (study group: 90, control group: 124) and included in the study. Toilet training in the study group was provided with professional support. In the control group, training was given by the families. Results: The age of training onset was 25.60 months in the study group and 24.19 months in the control group. The duration of bladder, bowel control and toilet training completion in the study group was spread over time, and it was frequently between 1-30 days in the control group (p=0.001). Conclusion: Our study showed that it would be appropriate to start training around 24 months and the process would be completed within 2 months. Especially in families that received professional support, the training processes of children were more positive.

Objective: Despite varying presentations of acute pancreatitis (AP) in children, the diagnosis has been increasing in recent years due to increased awareness. We aimed to identify the relationship among etiology, presentation symptoms, treatment response and complications of the children with acute pancreatitis. Material and Methods: Thirty children diagnosed with AP were evaluated for clinical and laboratory findings, treatment approaches, complications in the tertiary children hospital retrospectively. Results: The mean age of the patients was 12.4 ± 4.3 years and 60% were male. The drugs (30%), biliary tract diseases (26.7%), infections (16.7%), hyperlipidemia (10%) were the main causes of AP, in 10% of patients no etiological factor was detected. Abdominal pain (83.3%), nausea (70%), loss of appetite (63.3%), vomiting (56.7%), and fever (20%) were the most common symptoms. Ultrasonography, abdominal tomography and magnetic resonance cholangiopancreatography revealed pancreatitis related changes 63.3%, 85%, 70% of patients, respectively. Oral feeding was started on median 4 days (1-30), with polymeric diet (30%), and medium chain triglyceride rich enteral diet (70%). The median length of hospitalization (LOH) was 16.5 days (4-66). The patients fed with polymeric diet had a shorter hospitalization duration (p&amp;lt;0.036). The delayed initiation of oral feeding caused longer LOH (p&amp;lt;0.001). Conclusion: Consequently, this study underlines the children with acute abdominal pain, especially who use drugs like asparaginase and valproic acid, or that are known to have gallstone/biliary sludge, need to be examined for acute pancreatitis through pancreatic enzymes and ultrasonography. Moreover, the study also highlights that early feeding in acute pancreatitis is related with shorter hospitalization duration.

Acute Pancreatitis in Children: Neither to be Underestimated Nor to be Overlooked

儿童急性胰腺炎：不容小觑亦不可忽视

Amaç: Yeme bozuklukları (YB), sıklıkla tıbbi komplikasyonların eşlik ettiği psikiyatrik hastalıklardır. Bu çalışmada, YB hastalarının hastaneye başvuru sırasındaki komplikasyonları ve ilk tanısal başvuru değerlendirmesindeki vücut ağırlığı (VA) ile ilişkisi değerlendirilmiştir. Gereç ve Yöntemler: Adolesan Sağlığı ve Çocuk Psikiyatrisi değerlendirilmesi ile DSM-5 tanı kriterlerine göre YB tanısı almış hastalar çalışmaya dahil edilmiştir. Hastaların bulguları dijital tıbbi kayıt sisteminden elde edilmiştir. Tıbbi komplikasyonlarla VA ilişkilerini analiz etmek için vücut kitle indeksi (VKİ), VKİ persentili ve boya göre ideal VA yüzdesi (%İVA) hesaplanmıştır. Bulgular: Çalışmamıza 140’ı (%97.2) kız, 4’ü (%2.8) erkek olmak üzere 144 hasta dahil edildi. Hastaların; 94’ü Anoreksiya Nervoza (AN), 28’i Atipik AN ve 17’si Bulimia Nervoza (BN) olarak değerlendirildi. Hastaların %29.3’ünde (39) en az bir kardiyak, %54.2’sinde (78) gastrointestinal komplikasyon, %18.2’sinde (26) hipoglisemi ve %34.5’inde (48) sekonder amenore saptandı ve %45.8’i (66) ise hastaneye yatırıldı. İVA&amp;lt;%75 olan hastalarda hipoglisemi, bradikardi, düşük serbest triiodotironin (sT3), amenore ve hastaneye yatış oranları İVA≥ %75 olanlara göre istatistiksel anlamlı fazlaydı (p&amp;lt;0.001). Kemik mineral yoğunluğu (KMD) Z-skoru, YB tanısına kadar geçen süre ve amenore süresi ile ters ilişki olduğu saptandı (sırasıyla p=0.006; p=0.044; p=0.032). Sonuç: Bulgularımız, YB hastalarında ilk değerlendirmede sıklıkla en az bir tıbbi komplikasyonun geliştiğini göstermektedir. Genellikle ergenlik döneminde başlayan AN ve BN hastaları sıklıkla medikal şikayet ile pediatri polikliniklerine başvurabilirler. Bu hastalarda ciddi komplikasyonların önlenmesi için dahili hekimlerin YB erken tanısında rolü önemlidir.
