The purpose of this report is to describe chiropractic treatment of lower back and unilateral leg pain in a pregnant patient. A 26-year-old woman in her second trimester of pregnancy had severe pain in her lower back that radiated to her hips bilaterally and to her right leg. She reported tingling down her right lower leg to the dorsum of her foot. Although no diagnostic imaging was performed, her differential diagnoses included lumbalgia with associated radiculopathy. Treatment consisted of manual traction in the side-lying position using a specialized chiropractic table and treatment technique (Cox flexion-distraction decompression) modified for pregnancy. Relief was noted after the first treatment, and complete resolution of her subjective and objective findings occurred after 8 visits. When modified, this chiropractic technique appears to be an effective method for treating lower back pain with radiation to the leg in a pregnant patient who cannot lie prone.

Chiropractic treatment of a pregnant patient with lumbar radiculopathy

Intraclass correlation coefficient (ICC) is a widely used reliability index in test-retest, intrarater, and interrater reliability analyses. This article introduces the basic concept of ICC in the content of reliability analysis.There are 10 forms of ICCs. Because each form involves distinct assumptions in their calculation and will lead to different interpretations, researchers should explicitly specify the ICC form they used in their calculation. A thorough review of the research design is needed in selecting the appropriate form of ICC to evaluate reliability. The best practice of reporting ICC should include software information, "model," "type," and "definition" selections.When coming across an article that includes ICC, readers should first check whether information about the ICC form has been reported and if an appropriate ICC form was used. Based on the 95% confident interval of the ICC estimate, values less than 0.5, between 0.5 and 0.75, between 0.75 and 0.9, and greater than 0.90 are indicative of poor, moderate, good, and excellent reliability, respectively.This article provides a practical guideline for clinical researchers to choose the correct form of ICC and suggests the best practice of reporting ICC parameters in scientific publications. This article also gives readers an appreciation for what to look for when coming across ICC while reading an article.

A Guideline of Selecting and Reporting Intraclass Correlation Coefficients for Reliability Research

To describe and discuss the process used to write a narrative review of the literature for publication in a peer-reviewed journal. Publication of narrative overviews of the literature should be standardized to increase their objectivity.In the past decade numerous changes in research methodology pertaining to reviews of the literature have occurred. These changes necessitate authors of review articles to be familiar with current standards in the publication process.Narrative overview of the literature synthesizing the findings of literature retrieved from searches of computerized databases, hand searches, and authoritative texts.An overview of the use of three types of reviews of the literature is presented. Step by step instructions for how to conduct and write a narrative overview utilizing a 'best-evidence synthesis' approach are discussed, starting with appropriate preparatory work and ending with how to create proper illustrations. Several resources for creating reviews of the literature are presented and a narrative overview critical appraisal worksheet is included. A bibliography of other useful reading is presented in an appendix.Narrative overviews can be a valuable contribution to the literature if prepared properly. New and experienced authors wishing to write a narrative overview should find this article useful in constructing such a paper and carrying out the research process. It is hoped that this article will stimulate scholarly dialog amongst colleagues about this research design and other complex literature review methods.

Writing narrative literature reviews for peer-reviewed journals: secrets of the trade

The purpose of this study was to compare Functional and Kinetic Treatment with Rehabilitation (FAKTR) combined with cryotherapy to cryotherapy alone in the treatment of acute grade I or II inversion ankle sprains.This prospective, randomized clinical trial of adult (18-40 years of age) participants (n = 40) with acute grade I or II inversion ankle sprain of less than 3 weeks, who were randomly allocated into a FAKTR and cryotherapy group (n = 20) or a cryotherapy only group (n = 20). The participants had 3 treatments (inclusive of the initial consultation), with a fourth as a measurement follow-up (2 weeks after the third treatment). Measurement procedures were completed at the outset of the first to third consultations and the fourth measurement only consultation. Clinical measures taken by a blinded research assistant included the Numerical Pain Rating Scale, Foot function index, algometer, digital inclinometer for ankle dorsiflexion range of motion measures, the figure-of-8 maneuver measured swelling, and the Stork-Balance-Stand Test.Significant intergroup differences were observed for pain rating (P ≤ .01; 95% confidence interval [CI] -4.74 to 0.86), pain pressure threshold (P ≤ .05; 95% CI -1.06 to 1.52), balance and proprioception (P ≤ .01; 95% CI -5.28 to -1.39), and foot function index (P ≤ .01; 95% CI -30.12 to 4.83). No significant intergroup differences were observed in ankle dorsiflexion range of motion (P = .242; 95% CI -3.17 to 1.20) and edema measurements (P = .602; 95% CI 0.41-1.46).The FAKTR instrument assisted soft tissue mobilization treatment combined with cryotherapy indicated a trend toward greater clinical effectiveness than cryotherapy for measures of pain, pain pressure threshold, balance and proprioception, and foot function index; however, these outcomes were not reflected for ankle dorsiflexion range of motion and edema measurements.
