OBJECTIVES. To identify the learning needs of patients with cardiac disease and the aspects of the disease and anesthetic and surgical procedures about which Brazilian patients have the greatest gaps in knowledge. METHODS. Eighty preoperative patients answered a General Evaluation Questionnaire, a Questionnaire to Evaluate Patient Knowledge, and the Mini‐Mental State Exam. FINDINGS. Fifty‐nine patients had learning needs. More than 50% of the patients were mistaken or unable to answer questions about the disease, and the goals of and type of surgery to be performed and anesthesia to be used. CONCLUSIONS. Most patients had poor performance on the questionnaire that assessed their knowledge about coronary artery disease and its treatment. PRACTICE IMPLICATIONS. This study can contribute to health professionals' assessment of patients' knowledge. OBJETIVOS. Identificar as necessidades de aprendizagem de pacientes com doença cardíaca e os aspectos da doença arterial coronariana e da revascularização do miocárdio do nos quais os pacientes brasileiros apresentam conhecimento deficiente. MÉTODO. Oitenta pacientes responderam o Questionário para avaliação geral, o Questionário para avaliar o conhecimento e o Mini‐Exame do Estado Mental. RESULTADOS. Cinqüenta e nove pacientes apresentaram necessidade de aprendizagem. Mais de 50% dos pacientes erraram ou não souberam responder as questões referentes ao nome da doença, aos sinais e sintomas de complicação da doença, aos objetivos e tipo de cirurgia e anestesia. CONCLUSÃO. Muitos pacientes não apresentaram bom desempenho no questionário para medir conhecimento em relação à Doença Arterial Coronária e seu tratamento. IMPLICAÇÕES PRÁTICAS. Este estudo poderá contribuir para a avaliação do conhecimento dos pacientes por profissionais da saúde.

Deficient Knowledge Nursing Diagnosis: Identifying the Learning Needs of Patients With Cardiac Disease

The purpose of this article was to define and describe the fundamental aspects of similarity with application to the use of nursing terminologies.Data were obtained from Google, Cumulative Index to Nursing and Allied Health Literature (CINAHL), PsychINFO, and PubMed using the keywords "similarity views,""similarity,""concepts and categorization," and other published sources.Three prominent similarity views were compared, contrasted, and applied to the use of nursing diagnoses.Each view has intentions (requirements) that guide the categorization of information to concepts and influence naming of nursing concepts.By understanding similarity, nurse educators and technology designers can influence how nursing concepts are represented.

Identifying Nursing Concepts: Are We Similar?

The majority of nursing programs identify the nursing process and diagnosis as critical elements in their curricula, yet it is often absent in classroom and clinical discussions. The failure of faculty to integrate the nursing process and diagnoses into learning experiences creates students spending endless hours creating a care plan document that requires little critical thinking, a document that does not improve the student's likelihood of utilizing nursing diagnosis after graduation.This paper would outline the educational barriers for integration of these concepts into practice.Classroom and clinical strategies that guide the student to focused learning on nursing diagnoses and utilize standardized predicted care plans can produce desired curriculum outcomes, and ultimately, practice utilization will be presented.

Invited Paper: Teaching Nursing Diagnosis to Increase Utilization After Graduation

PURPOSE. To evaluate how the nursing process has been registered at a Brazilian teaching hospital. METHODS. Descriptive and retrospective study of 68 medical records. FINDINGS. The data collection of history was more frequent on the admission day. The documentation of physical exam was prioritized. Sometimes there were records of nursing interventions but not of nursing diagnoses. CONCLUSIONS. It was found some failures in the record of some steps of the nursing process. Although all steps had been used, they were not carried out consistently. IMPLICATIONS FOR NURSING PRACTICE. Some aspects deserve to be considered to improve the use and registration of all steps of the nursing process as to develop specific protocols, redesign the formulary of data record with careful and equal consideration of all steps, and empower all members of the nursing staff to implement the nursing process more effectively. OBJETIVO. Avaliar como o processo de enfermagem foi registrado em um hospital de ensino Brasileiro. MÉTODOS. Estudo descritivo e retrospectivo de 68 prontuários. DADOS. A coleta de dados do histórico foi mais freqüente no primeiro dia de internação. A documentação do exame físico foi priorizada. Às vezes havia registro de intervenções de enfermagem, mas não de diagnósticos de enfermagem. CONCLUSÕES. Foram encontradas falhas no registro de algumas etapas do processo de enfermagem. Embora todos os passos tenham sido usados, não foram realizados de forma consistente. IMPLICAÇÕES PARA A PRÁTICA DE ENFERMAGEM. Alguns aspectos devem ser considerados para melhorar o uso e registro de todas as etapas do processo de enfermagem, como desenvolver protocolos específicos, reformular o formulário de registro de dados com uma cuidadosa e igual consideração de todas as etapas, e capacitar todos os membros da equipe de enfermagem para implementar o processo de enfermagem de forma mais eficaz.

Editorial: Change Is in the Air

PURPOSE: To propose a nursing model of contamination that describes the key features of contamination at a level of abstraction needed for clinical decision making. DATA SOURCES: Relevant literature on contamination, biopreparedness, and classic epidemiologic literature were reviewed and analyzed. DATA SYNTHESIS: A model of contamination was created along with a description of benefits of use in practice, education, and research. DISCUSSION: The nursing profession is called to respond to contamination incidents on a local, national, and global level. Achieving optimum health outcomes while managing contamination incidents is enhanced by nurses' use of a model that incorporates six elements to identify and name instances of contamination and select suitable outcomes and interventions.

A Nursing Conceptual Model for Contamination

PURPOSE: Understanding how nurse practitioners use clinical decision skills in practice has the potential to guide education programs to ensure safe, competent, and high quality advanced nursing practice. This study used the lens of social judgment theory to examine the clinical decision making of 60 nurse practitioners. METHODS: A three‐group pretest–posttest comparison design was used to evaluate how much insight nurse practitioners had into their clinical decision‐making process. FINDINGS: Nurse practitioners had modest insight into their clinical decision‐making process. CONCLUSION: Self‐insight has implications for enhancing nurses' decision making, improving education, and fostering agreement among advanced practice nurses. IMPLICATIONS FOR NURSING PRACTICE: Self‐insight has implications for developing professional decision making and promoting appropriate educational opportunities for advanced practice nurses.

How Nurse Practitioners Make Decisions Regarding Coronary Heart Disease Risk: A Social Judgment Analysis

PURPOSE. The purpose of this case study is to demonstrate use of the nursing process and the standardized nursing languages of NANDA International (NANDA‐I), the Nursing Outcomes Classification (NOC), and the Nursing Interventions Classification (NIC) to assist a young male with paranoid schizophrenia to deal with auditory hallucinations. DATA SOURCES. Data were obtained from the experience and expertise of the author and published literature. DATA SYNTHESIS. This case study demonstrates nurses' clinical decision making in providing care for an adolescent with mental illness. CONCLUSION. This case study provides the pertinent nursing diagnosis, patient outcomes, and nursing interventions for a young male with auditory hallucinations in paranoid schizophrenia. IMPLICATIONS FOR NURSING. The use of NANDA‐I, NOC, and NIC can provide the necessary framework for enhancing and improving the management of care with patients who experience auditory hallucinations in paranoid schizophrenia.

Case Study: A Young Male With Auditory Hallucinations in Paranoid Schizophrenia

PURPOSE: Concept analysis methodology by Walker and Avant (2005 ) was used to define, describe, and delimit the concept of perception. DATA SOURCE: Nursing literature in the Medline database was searched for definitions of “perception.” DATA SYNTHESIS: Definitions, uses, and defining attributes of perception were identified; model and contrary cases were developed; and antecedents, consequences, and empirical referents were determined. CONCLUSIONS: An operational definition for the concept was developed. IMPLICATIONS FOR NURSING PRACTICE: Nurses need to be cognizant of how perceptual differences impact the delivery of nursing care. In research, a mixed methodology approach may yield a richer description of the phenomenon and provide useful information for clinical practice.

Perception: A Concept Analysis

Recently, I have been working with my colleague Dr. Dorothy Jones reviewing the archive materials of NANDA-I at the Burns Library at Chestnut Hill, Massachusetts Campus of Boston College. In the middle of this work, I am in the process of working with Dr. Heather Herdman and our publishing team at Wiley Blackwell in renaming the journal. This is the last issue of the journal under its present name the International Journal of Nursing Terminologies and Classificiations. In 2012, the journal will be renamed the International Journal of Nursing Knowledge. As I move between these two worlds, I am struck by the commitment to the work of NANDA-I that so many of you demonstrate from founding members Kristine Gebbie and Mary Ann Lavin to the many present day members (who I will not mention by name because I am bound to overlook someone). As I look through the materials for the archives, I am impressed by the changes that have occurred and continue to be in process with this journal. The journal, an outgrowth of the North American Nursing Diagnosis Newsletter, was originally edited by Rose Mary Carroll Johnson (first and third editor) who along with Noreen Frisch (second editor) groomed it to become international and well-respected renamed International Journal of Nursing Terminologies and Classifications (IJNTC). My predecessor Dr. Georgia Whitley, the fourth editor, provided the opportunity for the journal to blossom—increasing the international visibility and the number of manuscripts per edition. Dr. Whitley also improved the NANDA-I news section of the journal. By the time I became the fifth editor, Dr. Whitley had set the stage nicely so that I could continue to make improvements with the journal, and as a result, during my tenure as editor, there have been a steady stream of changes. Case studies edited by Dr. Margaret Lunney are now a regular feature. NANDA-I news was so popular that there was a need to shift this section to the NANDA-I web page so as to not compromise the pages dedicated to scholastic articles. The aims and scope of the journal have been revised and will be published in the next edition and on the NANDA-I web page. The new cover design and title will be debuted in the next issue. The International Journal of Nursing Knowledge (IJNK) reflects the more broad-based focus of the work of NANDA-I. Changes in the editorial board, which will also be notable in the first issue of 2012 bring us in line with other similar prestigious journals. The appearance of the table of contents, the availability of the journal online and ahead of print, and the changed frequency (from four times per year to three), with an increased number of publications per issue are all changes that allow us to move forward to becoming a relevant, ranked, and scholarly journal. I want to thank you—the NANDA-I members—for your willingness to share your thoughts and ideas about the direction of the journal. As I sifted through old newsletters and older editions of the journal, I realize that it is a dynamic group of people who have been committed to the work of NANDA-I over many years that help to propel this journal forward. I am grateful to each of you as I am to my editor predecessors—Rose Mary, Noreen, Rose Mary (again), and Georgia—for the impetus you have provided and continue to provide to make this journal what it is and what it will become—thank you. In memory: Helen Ruth Carroll Cox, a NANDA-I member who served 2 years on the Board of Directors, died Sunday, July 24, 2011, at her residence. Dr. Cox was the primary author of two nursing textbooks—Computers in Nursing (1987 American Journal of Nursing Book of the Year) and Clinical Applications of Nursing Diagnosis, which is currently in its fifth edition. Dr. Cox is survived by her daughter, Deana, her adopted son, Cliff Rogers, one sister, Cynthia Rively, two nieces, Nicole (Joe) Ely and Denise Rively, a great niece Jade Ely, a great nephew, Brayden Ely, her aunt Betty McBride Barnett, and her husband Audvee Barnett. The family requests memorials be made to The Carroll Family Scholarship Fund at Texas Tech University Health Sciences Center's Anita Thigpen Perry School of Nursing, Lubbock, Texas 79430 or to the Humane Society of Lubbock.
