Zirconia specimens printed by vat photopolymerization: Mechanical properties, fatigue properties, and fractography analysis

A systematic search of the literature was performed to identify and characterize articles reporting the influence of removable partial denture (RPD) therapy on satisfaction and quality of life (QoL).The literature search for relevant articles published between January 1983 and March 30, 2013 was conducted using keyword searches of electronic databases and complimentary hand searches. The search strategy included the following keyword combinations (MeSH and free-text terms): fail, complication, surviv*, longevity, outcome, patient satisfaction or QoL, and denture or prosthes*, and partial and removable. Based on inclusion and exclusion criteria, articles focused on the effect of RPD therapy were reviewed and classified according to Strength-of-Recommendation Taxonomy (SORT) criteria.The combined search identified 997 publications; 198 duplicates were identified and removed, leaving 799 references for further evaluation. After abstract review, 209 articles were selected and read; 18 reports representing findings from 4002 patients were included. While two studies were prospective comparative investigations, the majority of studies represented cross-sectional and retrospective studies. Different instruments were used to measure oral health-quality of life (OHQoL) and satisfaction. Several investigations reported a significant association of satisfaction and QoL with either (a) age of the patient, (b) number of occluding dental units replaced, (c) replacement of anterior teeth, and (d) nature of the opposing arch.A paucity of detailed investigations concerning outcomes of RPD therapy was noted. Improvement in OHQoL or satisfaction following provision of RPDs was not consistently reported. There is little evidence supporting any association between patient-reported QoL or satisfaction with technical or biological parameters of therapy. Therapeutic success of tooth replacement using RPDs should be carefully considered and compared with alternatives.

Factors Influencing Removable Partial Denture Patient‐Reported Outcomes of Quality of Life and Satisfaction: A Systematic Review

To evaluate the periodontal conditions and integrity of abutment and non-abutment teeth of patients evaluated 7 years after insertion of the removable partial denture (RPD).Twenty-two patients (17 women, 5 men) were assessed at the moment of denture insertion and 7 years later. The following items were verified in each assessment: bleeding on probing (BP), probing depth (PD), gingival recession (GR), and mobility (M), comparing direct and indirect abutment teeth, and the teeth not involved in the denture design. Tooth integrity was also evaluated and classified as intact when no caries or fractures were observed. The Kruskal-Wallis test was used to reveal statistical significance between the groups (p = 0.05) as well as the Bonferrroni-corrected Mann-Whitney test for post hoc comparison. The Wilcoxon test was used for evaluation within the group over time. Fisher's exact test was applied to cross data about abutment integrity.Statistically significant differences were found for GR (baseline, p < 0.001; 7 years, p < 0.001) and PD (baseline, p = 0.001; 7 years = 0.004) between the three groups at baseline and after 7 years of follow-up. Mean BP and M values increased from initial assessment to after 7 years of RPD use in every group, but no statistically significant difference was found between the groups. For abutment integrity, a statistically significant difference (p = 0.028) was observed, and the direct abutment exhibited more (33.3%) caries and fractures.RPDs generated more periodontal damage to direct abutments, since higher gingival recession probing depth indexes, and presence of caries and fractures were observed in comparison to indirect abutments and non-abutments.

Periodontal Conditions of Abutments and Non‐Abutments in Removable Partial Dentures over 7 Years of Use

Abstract Purpose Removal of ceramic restorations and appliances can be time consuming, invasive, and inconvenient. Erbium lasers offer an alternative noninvasive method for debonding of ceramic appliances. This paper aims to provide a comprehensive review of current literature on the effectiveness of erbium lasers for removal of ceramic restorations and appliances from natural teeth and dental implants. Methods A comprehensive search of 7 databases, including Medline (Ovid), Embase, Dentistry and Oral Sciences Source (DOSS), Web of Science, Cochrane Library, and ProQuest Dissertations and Theses was performed. The inclusion and exclusion criteria were agreed prior to the literature search. Two reviewers independently screened the title and abstract. A third reviewer then broke the tie, if any. The selected articles then underwent full text review and the data was extracted. Results The search identified 4117 unique articles published through June 10, 2021. Studies were assessed and categorized based on the type of restoration/appliance, type of abutment, type of laser, laser settings, efficacy of debonding, and pulpal temperature rise. Thirty‐eight full‐text articles were reviewed for inclusion. Time for ceramic debonding varies depending on the type of restorations and materials. Removal of zirconia crowns from teeth and implant abutments requires a longer period of time compared to lithium disilicate crowns. Temperature increases were reported as 5.5 degrees or less. Laser setting and laser type affect the debonding time and the increase in temperature. Examinations of debonded ceramics demonstrated no known structural damages resulting from laser applications. Conclusions Erbium lasers are effective noninvasive tools to remove all ceramic restorations/appliances from natural teeth and implant abutments without causing harm to abutments. Laser‐assisted debonding should be considered as a viable alternative to rotary instrumentation for ceramic crowns; however, clinical studies of erbium‐assisted ceramic retrieval are needed.

Erbium laser‐assisted ceramic debonding: a scoping review

Abstract Purpose Removal of cement‐retained implant crowns can be difficult and often requires sectioning of the prosthesis by rotary instruments. This study aimed to measure how much time is required in crown removal and the temperature changes when erbium‐doped yttrium aluminum garnet (Er:YAG) laser was used to retrieve lithium disilicate crowns from titanium implant abutments luted with composite resin (CR) cement and resin‐modified glass ionomer (RMGI). Materials and Methods Forty identical lithium disilicate crowns were fabricated for prefabricated titanium abutments. CR and RMGI cements were used to lute the crowns, 20 specimens for each cement. Specimens were kept in 100% humidity for 48 hours. Er:YAG laser was then used to facilitate the crown retrieval. The retrieval time was recorded. The temperature changes at the abutment level for each type of cement were recorded during irradiation of 10 specimens for each type of cement from 1 to 10 minutes. Data were analyzed using t‐test (ɑ = 0.01) and paired t ‐test (ɑ = 0.05). The surfaces of the crown and the abutment were further examined using scanning electron microscopy (SEM). Results The average times of crown removal from titanium abutments were 196.5 seconds for CR and 97.5 seconds for RMGI groups with statistical significance ( p &lt; 0.001). The temperatures measured from 1 to 10 minutes of irradiation ranged from 18 ° to 20.8 ° for CR and 18 ° to 23 ° for RMGI at the abutment surface, and 22.1 ° to 24.6 ° for CR and 22 ° to 24.8 ° for RMGI at the crown surface. No statistical differences were observed between temperature changes at the abutment or the crown for each cement ( p = 0.63); however, there was a statistically significant difference between the temperatures at the abutment and crown for both cements ( p &lt; 0.001). SEM examination showed no visible damage caused by treatment with Er:YAG laser. Conclusions It is faster to remove lithium disilicate crowns from titanium implant abutments when luted with RMGI compared to CR cement. The temperature rise was higher in the crown compared to the abutment. The type of cement had no effects on temperature changes. Heat generated from Er:YAG irradiation does not appear to be high enough to have any adverse effect on implant osseointegration.

In Vitro Examination of the Use of Er:YAG Laser to Retrieve Lithium Disilicate Crowns from Titanium Implant Abutments

Abstract Purpose To measure the effect of daily immersion in varying beverages (distilled water, tea, carbonated beverage, and orange juice) on surface roughness and color stability of 3D‐printed denture base resins. Materials and Methods 160 acrylic resin discs (15 × 2.5 mm) were made from heat‐polymerized (HP), and 3D‐printed resins (FormLabs, ASIGA, and NextDent). Resin discs ( N = 40) were immersed in different solutions (distilled water, tea, carbonated beverage, or orange juice; ( n = 10/group)). Surface roughness ( R a , µm) was measured initially ( T 0 ), 6 months post immersion ( T 1 ), and 12 months post immersion ( T 2 ) using a non‐contact profilometer. Color change (∆ E 00 ) was calculated using CIEL2000 at T 1 and T 2 using a spectrophotometer. The data were analyzed using Mann‐Whitney U test and Kruskal‐Wallis test in addition to general linear modeling ( α = 0.05). Results Significant differences in surface roughness ( R a ) were found between the baseline and immersion in distilled water, carbonated beverage, and orange juice ( p &lt; 0.05). HP resin showed increased roughness with all immersion solutions compared to T 0 with the highest value seen at T 1 with tea. ASIGA resin had the highest mean R a of 1.78 (0.43) after carbonated beverage immersion, while FormLabs resin had the lowest mean R a of 0.88 (0.06) after distilled water immersion. Tea immersion did not significantly affect R a among the resins ( p &gt; 0.05). Noticeable color changes were observed after immersion in tea, carbonated beverage, and orange juice. All resins exceeded perceptibility and acceptability thresholds at T 1 and T 2 . The combined effect of time and resin on color change was statistically significant ( p &lt; 0.001), while the combined effects of time/immersion solution, immersion solution/resin, and immersion solution/resin/time were not. Conclusion Carbonated beverage, tea, and orange juice increase the surface roughness of 3D‐printed resins. Tea, carbonated beverage, and orange juice resulted in noticeable color changes of HP and 3D‐printed denture base resins and this change was time‐dependent.

Effect of daily immersion of different beverages on the surface roughness and color stability of 3D‐printed denture base resins

Abstract Purpose To assess the flexural strength (FS), surface roughness, and color stability of additive manufactured (AM) and subtractive manufactured (SM) denture teeth materials, as well as the effect of thermocycling on these properties. Materials and Methods Eighty strips (20/material type; 64×10×3.3 mm) and 60 discs (15/material type; 12×4 mm) were fabricated from four different denture teeth materials (Straumann, Flexcera, Ivoclar, and Candulor). The specimens were divided into two subgroups (nonthermal cycled and 5000 thermal cycled,10 per subgroup), and subjected to a 3‐point test for FS using a universal testing machine at a crosshead speed of 5.0 mm/min. Surface roughness (R a , µm) and color differences (Δ E 00 ) of disc specimens were measured before and after thermocycling using a noncontact optical profilometer and a spectrophotometer, respectively. The CIEDE2000 formula was used to calculate the color differences (Δ E 00 ) before and after thermocycling. Data were analyzed using 2‐way ANOVA followed by post hoc multiple comparison tests ( α = 0.05). Results There was no significant difference in flexural strength between nonthermal cycled SM materials ( p = 0.15), which were significantly higher than AM materials ( p &lt; 0.001). The flexural strength values of all tested materials were significantly reduced after thermocycling ( p &lt; 0.05). The material type had a significant effect on the R a values ( p &lt; 0.001), whereas thermocycling did not ( p = 0.81). After thermocycling, all materials exhibited color changes lower than the clinical perceptible threshold (Δ E 00 = 2.7). Conclusions Thermocycling decreased the FS of denture teeth materials manufactured with AM and SM, but did not affect the surface roughness. The FS values of SM groups were significantly higher than AM materials, irrespective of thermocycling. No clinically significant color changes of the denture teeth material were noted.

Abstract Purpose Cell adhesion and subsequent proliferation on material surfaces depend on the physical and chemical characteristics of the material. There is a lack of literature on human gingival fibroblast proliferation on comparatively newer additively manufactured materials like silicon nitride. This study focused on the physical characteristics of the materials with the aim to compare the adhesion and proliferation of human gingival fibroblasts on additively manufactured silicon nitride (AMSN) with additively manufactured zirconia, conventional milled titanium (MTi), and milled zirconia. Materials and Methods Surface roughness and water contact angle were measured by profilometer and goniometer, respectively. CCK‐8 assay was done to assess the cell growth at 24 h (day 1), 48 h (day 2), and 72 h (day 3) in the same well. The morphologies of fibroblasts after cell attachment and proliferation were evaluated using scanning electron microscopy (SEM) after 72 h. Results At the end of 24 h (day 1) additively manufactured zirconia showed the best proliferation among the experimental groups, which was around 50% of the positive control group proliferation. There was no statistically significant difference among the experimental groups. At 48 h (day 2) and 72 h (day 3), a loss of cell growth was seen in almost all the experimental group wells. A positive cell proliferation on the AMSN was observed on day 3. Conclusion Comparable cell proliferation was observed in the experimental groups. No conclusive correlation could be drawn between cell proliferation and surface roughness and water contact angle values in the experimental groups.

Comparative evaluation of human oral fibroblast proliferation on 3D‐printed zirconia and silicon nitride as new ceramic materials for implant abutment

Abstract Purpose This narrative review aims to elucidate the anatomical features of sleep‐disordered breathing (SDB) in children. By identifying key structures and intervening proactively, we seek to alter craniofacial growth patterns and improve functional outcomes for SDB children. Methods The literature on pediatric sleep‐disordered breathing (PSDB), pediatric obstructive sleep apnea (OSA), anatomical predispositions, and the relationship between skeletal deformity and PSDB was examined using PubMed and Google Scholar databases, covering studies from 2006 to 2024. Results Forty relevant articles were reviewed, focusing on craniofacial characteristics associated with PSDB. The etiology of PSDB is multifactorial, with adenoid and palatal tonsil enlargement being the most common cause. While adenotonsillectomy is recommended as the primary treatment, residual SDB may result in craniofacial skeletal deformities contributing to upper airway constriction. Typical craniofacial phenotypes of SDB include excessive vertical growth and constriction of the maxilla, a retruded mandible, and posterior rotation, known as a Class II hyperdivergent pattern. Conversely, Class III with an underdeveloped maxilla shows a relatively lower risk for SDB due to reduced nasal cavity and nasopharyngeal airway volumes. Transverse maxillary constriction with a high, deep palatal vault is a significant risk factor. Additionally, nasal obstruction and low tongue posture, with or without a short lingual frenulum, are identified as craniofacial risk factors for SDB development in children. Conclusion Early diagnosis and intervention are critical in managing PSDB. Dentists, through screening and early treatment, can significantly influence craniofacial growth and health outcomes. A multidisciplinary approach is essential for effective management, improving the quality of life and long‐term health of affected children.
